1.)    Does your Institution do laboratory safety inspections?

Yes. I work for a clinical laboratory in a healthcare system.

2.)    What areas are covered (general safety, chemical safety, biosafety, etc)?

The areas that we cover are general safety, chemical safety, biosafety (especially bloodborne pathogens), and knowledge of safety procedures.

3.)    How often?

Individual hospitals often inspect twice per year. We also conduct our own internal lab safety survey of each site twice per year. Every two years our accrediting agency, the College of American Pathologists, conducts a detailed site survey that includes safety.

4.)    How did you justify the need for these inspections to leadership?

Nobody has a choice. These are all required by our accrediting agencies - The Joint Commission, College of American Pathologists and so forth.

We have a general safety checklist which we do every time and then a routing checklists with each inspection. The routing check list is Chemical, biological, Controlled substances, and fire and general  safety

3.)    How often?
We traditional completed quarterly  lab inspections of every lab on campus, but last year we started a risked based lab inspection program. The labs are inspected yearly, twice a year, or quarterly and their inspection schedule is based on hazards in the space and the history of compliance with the program. We call them Tier assignments and we have tier, 1, 2, & 3

4.)    How did you justify the need for these inspections to leadership?
Our  Chemical Hygiene plan requires inspections
